Parts A & B: Required for all youth and adult participants at any Troop event (day or overnight).
Signatures: Both youth and parents must sign the form on Page A.
Restrictions: On Page A, list any participant restrictions or check "None".
Medication Preference: On Page B2, select preference for the administration of non-prescription medication and sign that section.
Immunizations: On Page B2, indicate Tetanus immunization date. Must be within the last 10 years.

Health Insurance Card: Include a photocopy of both the front and back of your Scout’s current health insurance card.
Part C: Required in addition to A & B for events lasting 72 hours or longer, such as Extravaganza and Summer Camp. Requires an annual physical and a licensed medical provider's signature. Forms Link: All Units
Validity: All forms are valid for 1 year from the signature date and must be renewed annually.
Assembly: Please staple the pages together. Do not fold the forms, as flat documents are much easier to file.
Submission: Please do not send this information to us digitally. The Troop accepts hard copies only. Turn them in at a Troop meeting in the Forms Box in the church foyer.
Simplified Paperwork: We use a single online Annual Waiver (Scout and Adult versions) to replace event-by-event paperwork.
Annual Renewal: You only need to complete the form once per calendar year when joining or re-registering.
Quick Checkout: When registering for outings in the Troop Store, simply check the box confirming you have a current waiver on file and agree to its terms.

Additional Waivers: Special events, venues, or camps may require additional waivers. These will be sent via email and turned in at a Troop meeting.
